Habit of the Heart Foundation Grant Making Guidelines

Purpose:  The purpose of the Habit of the Heart Fund is to provide assistance to Hendricks County children, women and families in emergent need.

Criteria:

  1. Children, women or families residing in Hendricks County.
  2. Requests may only be submitted by representatives from Hendricks County Schools (nurses, guidance counselors, teachers, etc.) or from Family Promise of Hendricks County. Other third-party representatives (other non-profits, vendors, etc.) shall be directed to make requests through school representatives.
  3. The proposed grant recipient cannot have a “significant relationship” with a Habit of the Heart Grant Committee member.
  4. Eligible items for funding are:
    1. Dental work
    2. Eyeglasses
    3. Medication (with a 90-day limit for funding)
    4. Hearing aids
    5. Clothing or shoes
    6. Other emergent needs
  5. Ineligible items for funding include:
    1. Rent
    2. Utilities
    3. Contact lenses
    4. Daily transportation or car payments
    5. Bill payments or bills in arrears
    6. Childcare
    7. Scholarships
    8. Addiction care
    9. School fees or educational materials
  6. No grants will be made to benefit the same recipient within 12 months
  7. Items not listed as eligible or ineligible will be considered on a case by case basis. Any grants recommended outside of these guidelines will be documented during consideration.
  8. These guidelines shall be reviewed annually, and any changes confirmed by August 31.

IMPORTANT: When requesting assistance for multiple family members, complete one form for each Individual and indicate the total number of members (see below).

NOTE: Requests for rent or utilities will not be considered, but Family Promise may be able to assist.

Requests will be considered withdrawn if there is no activity after 90 days of the original request, e.g., doctor appointments not completed, prescriptions not picked up. To reactivate the request, a new request must be submitted from you to the Habit of the Heart.
